) -Pass level 2 background - Knowledge of budgeting and financial management - Experience working with individuals with behavioral health needs - Strong communication and interpersonal skills - Ability to work independently and as part of a team - Excellent organizational and time management skills Office is located in haies city will need to ensure in office at lease once a week and meet requirements for clients.
Clients require home visits based off living setting with the most being once monthly.
Duties:
- Provide support and assistance to individuals with behavioral health needs - Develop and implement individualized service plans - Conduct assessments to determine eligibility and level of care - Coordinate and monitor services provided by various providers - Assist individuals in accessing community resources and support networks - Advocate for the rights and needs of individuals served - Maintain accurate and up-to-date documentation Note:
